On May 21, 2024, the Ministry of Health issued Decision No. 1332/QD-BYT on the Electronic Health Book serving integration on the VNeID application.
Accordingly, the Electronic Health Book serving integration on the VNeID application includes the following contents:
- Administrative information includes: Full name; Date of birth; Gender; Ethnicity; Nationality; Occupation; Phone number; Personal identification number/ CCCD, Date of issue, Place of issue; Address of residence; Health insurance card number, Initial medical examination and treatment registration place; Card validity period; Name of representative, Personal identification number/ CCCD, Relationship; Phone number.
- History (synthesized from historical medical examination and treatment data): Allergy history, Disease history, Vaccination history.
- Information of medical examination and treatment period (reported when the medical examination and treatment period ends): Hospital code; Hospital name performing medical examination and treatment.
+ Information of medical examination and treatment period: Reason for medical examination; Hospital code; Hospital name performing medical examination and treatment; Date of medical examination; Form of medical examination and treatment; Date and time of hospital admission; Date and time of hospital discharge; Treatment result; Discharge status; Hospital code; Hospital name where the patient will be transferred to.

The content of the “Electronic Health Book” is established, digitally signed by the medical examination and treatment facility, and integrated on the VNeID application.
The “Electronic Health Book” is applied to pilot for patients, used at all public and private medical examination and treatment facilities and other needs of the people in accordance with the provisions of the law.
The standard and data exchange format of the Electronic Health Book is implemented according to the standard and output data format regulations serving the management, appraisal, payment of medical examination and treatment costs and settlement of related regimes. The display of information content in the Electronic Health Book on the electronic application ensures fullness, convenience when using, easy access, easy viewing, convenient lookup.
The decision takes effect from May 21, 2024.
